Once both results are ready, talk them through over a video consultation with an NMC registered (Nursing and Midwifery Council) fertility practitioner, with clear next steps.
5. Get your reports
Receive two comprehensive fertility reports covering both your sperm analysis and hormone profile, written to be understood without a medical degree.
Who it is for
This is the right test if you and your partner have been trying to conceive for 12 months or more, or if you simply want the most complete view of your fertility health in one go. Because hormones play a central role in how sperm are produced, pairing the two tests can reveal context a sperm test on its own would miss. It gives you a clear, accurate picture you can act on, with expert guidance to take the next steps with confidence. These tests are offered in line with national guidance.
What this male fertility test measures
This test goes further than a standard sperm test. The semen analysis covers nine key markers: semen volume, total count, concentration, progressive motility, non-progressive motility, total motility, immotile sperm, vitality, and morphology, all compared against World Health Organisation (WHO) reference ranges. The male hormone profile then measures eight hormones from a blood sample: total testosterone, free testosterone (calculated), FSH (follicle-stimulating hormone), LH (luteinising hormone), oestradiol, prolactin, SHBG (sex hormone binding globulin), and DHEA sulphate. Together these show how well your body is signalling and supporting sperm production, giving you and your clinician context that a semen analysis alone cannot.
How the kits work
Your order arrives as two separate kits: one for your sperm sample and one for your blood sample. Order before 4pm on a weekday and your kits arrive the next day. You collect your sperm sample at home and package it using the temperature-controlled kit, which is unique to Malebox and helps keep the sample stable in transit. Once both samples reach the lab, your results are brought together and delivered in a single video consultation.
Clinical-grade accuracy, explained clearly
Your samples are analysed by an andrologist who is HCPC (Health and Care Professions Council) registered, following WHO guidelines. Your combined results are then delivered in a video consultation with an NMC (Nursing and Midwifery Council) registered fertility practitioner, who explains what each result means and what your options are. You also receive a clear, easy to read written report, designed to be understood without a medical degree.
Key metrics from sperm sample
Semen volume
Semen volume
Semen volume refers to the total volume of your sample, before you added the preservative. A clinical threshold of 1.4ml is often referred to as ‘sufficient’.
Total count
Total count
This indicates the number of sperm present in the sample. Total count is the total number of sperm.
Concentration
Concentration
Concentration is the number of sperm per milliliter.
Progressive motility
Progressive motility
Progressive motility is the percentage of sperm swimming forwards in a straight line. The clinical threshold for this is 30%.
Non-progressive motility
Non-progressive motility
Non-progressive motility is the proportion of sperm that are moving but not forward and straight, i.e. they may be going backwards or in a circle or just wiggling on the spot.
Total motility
Total motility
Total motility is the combination of progressive and non-progressive sperm.
Immotile sperm
Immotile sperm
Immotile sperm is the proportion of sperm that are not moving at all.
Vitality
Vitality
Vitality measures the proportion of sperm cells that are alive. It assesses whether the sperm membrane is intact and hence whether it is a viable sperm. Vitality focuses on viability, indicating the ability of the sperm to fertilise an egg.
Morphology
Morphology
Morphology measurement assesses the shape of the sperm, this includes the head, mid-piece and tail. A normal form requires all parts of the sperm to be normal in appearance. It is very common to have a very low percentage of normal forms within your sample.
Key metrics from blood sample
DHEA sulphate
DHEA sulphate
A hormone made by the adrenal glands that supports testosterone production and overall hormone balance.
FSH (Follicle Stimulating Hormone)
FSH (Follicle Stimulating Hormone)
A hormone from the brain that tells the testicles to make sperm. It helps us understand how well sperm production is working.
LH (Luteinising Hormone)
LH (Luteinising Hormone)
Another brain hormone that tells the testicles to produce testosterone.
Oestradiol (E2)
Oestradiol (E2)
A form of oestrogen. Men need small amounts, but higher levels can interfere with testosterone, libido, and fertility.
Total Testosterone
Total Testosterone
The main male hormone, important for sex drive, energy levels, mood, muscle strength, and sperm production.
Free Testosterone (Calculated)
Free Testosterone (Calculated)
The amount of testosterone your body can actually use. This often explains symptoms better than total testosterone alone.
Prolactin
Prolactin
A hormone that can affect testosterone levels. If it’s high, it may impact sex drive and fertility.
SHBG (Sex Hormone Binding Globulin)
SHBG (Sex Hormone Binding Globulin)
A protein that binds to testosterone. If SHBG is high, less testosterone is available for your body to use; if it’s low, more is available.

What does the male hormone profile actually test?
What does the male hormone profile actually test?
Alongside a full semen analysis, this test measures eight hormones from a small blood sample: total testosterone, free testosterone, FSH, LH, oestradiol, prolactin, SHBG and DHEA sulphate. These hormones drive how sperm are produced, so reading them together gives a fuller picture than sperm alone.
How is this different from the standard at-home sperm test?
How is this different from the standard at-home sperm test?
The £240 test is a semen analysis on its own. This one adds the eight-hormone blood profile, making it the more complete option. If you and your partner have been trying for 12 months or more, NICE (National Institute for Health and Care Excellence) guidance points towards this fuller investigation.
